Million Hearts Hypertension Control Champion
Maple City Health Care Center was one of 15 national champions of the Million Hearts Hypertension Control Challenge in 2020. The Million Hearts Challenge identifies practices that have worked with patients to achieve a hypertension control rate of 80% or greater.
